Sintel (Albendazole) is a broad-spectrum benzimidazole anthelmintic, highly effective against most nematodes and several cestodes. It possesses vermicidal, ovicidal, and larvicidal actions. Its primary mechanism involves inhibition of tubulin polymerization, which disrupts cytoplasmic microtubule formation, leading to impaired glucose uptake and eventual parasite death.
Neonates (not recommended).
Pregnant women (should be avoided; insufficient data on placental transfer).
Caution in children <10 kg (dose reduction may be considered).
Mechanism of Action: Inhibits glucose absorption in helminths, depleting their energy stores, causing immobilization and death.
Absorption & Metabolism: Poorly absorbed from the GI tract; undergoes extensive first-pass metabolism in the liver.
Active Metabolite: Albendazole sulphoxide (half-life ~8.5 hrs) retains anthelmintic activity.
Excretion: Mainly via urine as metabolites.
Common: GI upset, headache, dizziness, mild liver enzyme elevation.
Less common: Rash, fever, reversible alopecia.
Rare: Blood disorders (leucopenia, pancytopenia), allergic shock (cyst rupture), neurological effects in cerebral involvement.
Adults & Children >2 years
Single dose (400 mg / 10 ml suspension):
Enterobius vermicularis, Trichuris trichiura, Ascaris lumbricoides, Ancylostoma duodenale, Necator americanus
Strongyloidiasis / Taeniasis: 400 mg daily for 3 consecutive days. Repeat after 3 weeks if needed.
Giardiasis: 400 mg once daily for 5 days.
Children (1–2 years): 200 mg single dose (5 ml suspension).
Children <1 year: Not recommended.
Hydatid Disease (Echinococcosis):
≥60 kg: 400 mg twice daily for 28 days.
<60 kg: 15 mg/kg/day in two divided doses (max 800 mg/day).
Cystic echinococcosis: Up to 3 cycles of 28 days, separated by 14 drug-free days.
Alveolar echinococcosis: Cycles may need to be continued for months/years.
Monitor blood counts and liver function tests before and during treatment.
Exclude pregnancy prior to initiating therapy.
Use under strict medical supervision in echinococcosis with regular lab monitoring.
Avoid during breastfeeding unless clearly necessary.
